Eligible candidates can send their applications in the prescribed format together with self attested copies of the documents/testimonials, two recent passport size photograph and one self addressed post card, two self addressed envelopes of size 12 cms X 25 cms out of which one should contain/affix postage stamp worth Rs.150/- and two postal slips indicating name and postal address, to Director of Health Services, Andaman & Nicobar Administration, Port Blair by post or should be dropped in the box kept in office.
